DHS and DIS Sports Physicals~ Monday, June 13
All students interested in participating in sports at DHS and DIS during the 2022-2023 school year must have a Physical Completed before participation. We are asking for a $10 donation to help offset any costs that may occur.

If you are interested in participating in this opportunity, please make sure to complete the following CIF Pre-participation Physical Evaluation (PPE) forms and bring them with you on June 13th:
- CIF PPE – History Form English (Completed by Parents/Guardians and must be signed)
- CIF PPE – History Form Spanish (Completed by Parents/Guardians and must be signed)
- CIF PPE – Pre-participation Physical Form
- CIF PPE – Medical Eligibility Form
- Assumption of Risk- English
- Assumption of Risk - Spanish
IMPORTANT: MAKE SURE ALL FORMS ARE SIGNED WHEN YOU ARRIVE FOR YOUR PHYSICAL
